Do you? We have no idea how much of the medication works. We know it works in some parts better than others through observations and assumptions.
If we actually knew medicine, we tackle pain, injuries and other ailments through localised medical intervention, not taking a pill and hoping for the best.
Dude, you're literally describing pharmacology.
Localized effects have always been the goal, but actually making a drug that gets only to its intended destination, does a very specific thing, and doesn't escape to the rest of the body is very hard, very expensive, very painful, or a combination of the three.
