Your scrotum doesn't retain its elasticity forever. As people get older, their scrotums will begin to sag and droop. Also (this doesn't apply to everyone), as you get older, your heart doesnt function as well as it used to, and this can cause fluid to pool (edema) in your lower half. The scrotum is one of the first places to begin accumulating fluid, causing it to not only sag, but also get significantly larger. This can lead to discomfort and injures.
With all that said, this thing is 110% fetish wear. Just wear briefs if your scrotum is super saggy

Might be more helpful for people with specific conditions. Off the top of my head i'm thinking Bell Clapper Defomity and Varicoceles. But even those cases, I'd think you would just…wear a jockstrap? Generally a good idea for intense physical activity, too, for everyone that has testes.
