Bottle water is in reference to giant corporations like nestle privatizing access to clean water.
These corporations usually end up buying up rights to an entire water source and restricting access from locals, even if locals had used the water for generations
Medicine is more about the markup of life saving drugs (at least how I interpreted it). Insulin costs less than 10 dollars to make, but gets marked up to 300 dollars. Insurance doesn't cover it all, so you end up paying most of it out of pocket.
