It does bring up a tough question though: what do we do with people who need treatment but refuse to accept it?

They're literally dinosaurs in the same way that humans and other vertebrates are literally fish. Which is to say you can describe them that way taxonomically, but the saying "birds are literally dinosaurs" is misleading and disingenuous.
