The first distro I used was SuSE (yes, before there was "Open" in the name π ) on a Pentium III
I spent many years on Arch, but it don't think it's the distro I used the most any longer...
The distro I am using now is Fedora Workstation, which has been my daily driver for a dozen years now, and it's also my favorite distro.
The distro I'll be using in the future may be Silverblue
The distro I hate the most... is Ubuntu π I had so many troubles over the years with this one, there was always something nonfunctional
My honorable mention: CachyOS looks pretty nice π
As for all the distros I tried... there's way too many π
When I first saw "Distros i've used:", I thought there's no way I could list that, after my prolific distro-surfing phase between 2004 to about 2014... I don't know how many tall spools of CDs and DVDs I filled with distros, before I switched to usb, trying even more, with even less footprint to gauge it by.
... But, by just simply following the outline provided, I arrive at:

Favourite/Current Distro/Distro I've used the most: Arch btw.

First Distro Used: Corel Linux (I am old lol)
Honourable Mention: Pop!OS with the COSMIC desktop is pretty nice considering how new it is IMO.
Future Distro: I've messed around with NixOS a bit and I kind of want to try it. I like to tinker a lot though so sometimes immutable OS-es kind of interfere with that.

First Distro Used: Not entirely sure. We had a couple Linux computers growing up that my dad set up and he distro hopped a bit. I used Ubuntu(or variants, e.g. xubuntu) during high school and a bit of college.
